Preventing cardiovascular disease

The George Institute Centre for Cardiovascular Health
The Centre employs a multidisciplinary staff and focuses on preventive care. The staff consists of specialists and other health care professionals working together to provide a comprehensive approach to vascular risk management.
The Centre utilises a proprietary software tool to assist in the evaluation and management of vascular risk. Developed by researchers at the Institute, the software synthesises patient data along with the many and complex existing medical guidelines to provide individualised evidence-based recommendations.
The basic service model for the Centre includes assignment of an allied health care professional as a "case manager", who will represent a focus for continuity of care as the patient is seen by relevant specialists and other health care professionals. Patients with acute problems related to their vascular health may be referred to external specialists for further treatment.
The Centre for Cardiovascular Health has been designed to provide an all-inclusive approach to vascular health management, including thorough and open communication with General Practitioners (GPs). GPs will receive information on the outcome of their patient’s visit, including any medical and lifestyle treatments prescribed and suggested follow-up. It is the goal of the Vascular Clinic to pursue a collaborative approach with referring GPs, in order to maximise opportunities for maintenance of optimum vascular health in their patients.
